What do I pay out of pocket for an eye exam if I have a Unity Vision’s Gold Plan?

As long as you stay In-Network, you only have to pay $15 out of pocket for your annual eye exam.

What lens options are covered under Unity Vision’s Gold Plans?

Unity Vision’s Gold Plans cover standard lens options like single vision, bifocal, and trifocal lenses In-Network. If you choose a Unity Vision Premium Network provider, progressive lenses are also covered.
