We’ve expanded the Unity Vision LASIK Discount Network to include some of the top LASIK surgery centers across the state. Most providers in our network offer a $1,000 discount to Unity Vision members, bringing you trusted care at a better value. In addition to the $1000 discount, Unity Vision gives you a $200 rebate in lieu of glasses or contacts when you use your Lasik benefit.
Providers in the network offer discounts on LASIK, PRK, ICL, and KAMRA inlay procedures. Don’t forget, you can also use your FSA or HSA Dollars to save even more.

What is the difference between a Standard Provider and a PLUS Plan Provider?
As a PLUS Plan Member, you can see any doctor in our network, but you are entitled to EXTRA Benefits if you choose a PLUS Plan Provider. Over 85% of our network are PLUS Plan Providers. The difference is these doctors have agreed to include extra items as seen above at no additional cost to you.
How can I get a Member ID Card?
It’s simple, you can log in to your Member Portal and print it directly from there.
Refer to the section above for the Member Portal. You can also call (855) 918-2020, and one of our Customer Service Representatives can send you a copy and give you your Member ID over the phone. It’s important to note, you Do Not need a card to see our providers. They have access to our online eligibility site and can locate your benefits with your first and last name, as well as your Date of Birth.
